We understand the importance of finding the right home and environment. We work collaboratively with you and your support network to identify and source the right setting for you. Whether that’s in your own home, in supported living or in a registered care home, you can rely on us for safe, flexible and personalised support in a setting that meets your needs.

We are experts in delivering innovative integrated health and social care, to ensure our clients achieve the best outcomes. We specialise in supporting people with the most complex needs and challenges in life who often have chaotic backgrounds including forensic histories, substance and alcohol use, mental illness, personality disorders many of whom are living with trauma.

We work closely in partnership with commissioners, mental health trusts, GPs, clinicians, local community groups and people in the criminal justice service, to help better understand the needs and identify the right support and care for our clients.

We have a breadth of experience and expertise especially around managing risk and enabling the hardest to reach to join in and make positive changes to their lives and the community in which they live.

Our main aim is to provide holistic person-centred support that ensures the true integration and co-production of services that work together to change lives.

We focus on five key areas with service users so that they have the best chance moving forward in a positive direction:

Economic Well-being

Each Service User takes their own road to achieving economic well-being. We provide support tailored to each person’s circumstances. Maximising income at an early stage of the support planning process begins to foster independence and recovery.

Staying Safe

We support people to address specific risk areas. Positive risk management and good decision making can keep both Service Users and the local community safe. It can be a gradual process as Service Users engage and develop trust in the support provided by our staff.

Achieving ambitions

We support our Service Users to achieve their ambitions and aspirations. These may include gaining basic skills such as literacy or social skills. We provide support to improve social links with family, develop positive social links with friends and develop roots within the community.

Healthy living

We support Service Users to access the primary healthcare they require and to take responsibility for their own nutrition and exercise. To achieve this, we work in partnership with a wide variety of specialist agencies. We also run events and programmes designed to foster healthy lifestyles.

Making positive contributions

Service Users begin contributing from the outset as services are co-produced with them. Expertise from Service Users is used across the organisation particularly in training staff and policy development. We also work in partnership with community,
