Outcome Focused Pathways

Many of the people we currently support through our outreach services have transitioned on from residential or supported living – now supported with minimal hours each week in their own home or to access community activities.

These people have worked through a pathway to independence and are now significantly more independent – having learnt coping mechanisms, increased their skills and confidence and developed links and networks in their local community.

Support is often focussed on developing learning opportunities, work ready skills and building ‘natural’ links and friendships in the community.

We often find that peer or unpaid support replaces our paid support within this setting overtime.
